Phase 2 - Gateway to Independent Living

Four Week Resettlement Programme

Our resettlement programme aims to equip clients with the skills that they will need to re-engage with the community and to live an independent life.  By using information, training, work experience and work shops the clients can develop both their skills and self confidence.

Structured Groups – Resettlement Phase ll

Covers independent living skills, budgeting, managing debt, maintaining a home and good tenant module, building a healthy support network, safety, time management and structure.

Compiling a Curriculum Vitae, Interview Techniques (role play), and education and training requirements.

Personal boundaries, relapse prevention, labelling and perceptions.

This programme is delivered by the Resettlement Team and a range of partner agencies including: Housing, college, Progress Z Work, Connexions to name a few.

The resettlement programme covers a wide variety of aspects such as housing, employment, debt and finances, drug and alcohol misuse. It also covers personal development, concern over family relationships and childcare as well as general issues of reintegration and independent living.

The programme is delivered through group workshops, one-to-one representation to resolve outstanding matters. It provides a structured context encouraging personal and life skills to meet the needs of the individual as additional support to previous and existing programmes that may be in place.

The programme aims to address individual needs, enabling clients to change their lifestyle and achieve a more stable environment and more satisfactory coping mechanisms.

In addition to the Resettlement Programme we also work with local employers and the County Council to provide work placement opportunities to individuals who have completed Phase 1 & 2.  The Resettlement Team work closely with service users to identify their long term goals and will then work towards finding a placement that suits the individual.  We have had work placements within the County DAAT, Animal Health Team, County Equipment Stores and we are always working with local employers to create more opportunities.  The aim of the work placement is to build confidence and self-esteem and to allow individuals to feel that they are able to contribute and make a difference.  These placements also allow individuals to prove that they are a valuable member of a team and gain workability skills to help them progress and move on into full time employment.

THERAPEUTIC AFTERCARE ON COMPLETION OF PHASE l & ll

We believe that recovery may need a lifetime of maintenance and support and that there are no short term solutions. That’s why we offer a one year post rehabilitation support programme for all clients successfully exiting rehabilitation. In practice we are always available to any client who has been through the programme.

Clients can attend the weekly “Aftercare Group,” facilitated by a therapist. Here they receive peer support and can discuss the stresses and strains of living in the community. They can book a one-to-one with a therapist or if they prefer, just drop in for a coffee.
